It sounds like a bad upper respiratory infection. These are pretty common with captive reptiles. The causes are numerous, mostly not keeping the turtle warm enough. Most turtles need a dry basking area of 87-90 degrees F. Tortoises need several degrees higher.
The best fix is to take the little guy to a vet. If his infection is bad enough his mouth is foaming he will most likely need antibiotics.
